While everyone was great, there were several who stood out above the rest. They were always optimistic and enthusiastic and offered me support when I needed uplifting. kitchen and dining room. Your staff have been wonderful. The cooks,
servers, and many others have worked so hard to see that we have
everything we need and given us a good variety of good food.

short time back it became necessary for me to spend a few days at your
facility for rehabilitation purposes. I would like to compliment you and
the people of your facility on the quality of care I received while I
was there. The building and all areas were very clean, neat and orderly.
The doctors, nurses and aides were so attentive and helpful that there
was nothing left to want. While being in the condition I was in is
certainly not desirable, if it were necessary, I cannot think of a
better place to be than the one you are operating.

1. HOW DO I ARRANGE ADMISSION TO YOUR SKILLED OR RESIDENTIAL FACILITIES?
For Friendship Health Center, our Skilled Nursing Facility, you are
generally admitted from a hospital and the Discharge Planner or Social
Worker at the hospital will make the arrangements for your admission. It
is a good idea to tour the facility first and then let the hospital
planners know that you are interested in coming here.
For the Oddfellows Home, our Residential/Retirement facility, it is best
to call the Administrator, Randy Mondello at (503) 872-1104 and arrange
for a tour or just drop by the facility and ask to be shown around Ms.
Mondello can answer questions regarding availability, payment options,
and services. If coming from a hospital, the discharge planner can help
make arrangements as well.
2. WHAT SHOULD I LOOK FOR IN A FACILITY WHEN SELECTING A CARE CENTER?
It is always best to vist a facility before making a selection. Any
facility should be eager and prepared to give you a tour. Common things
to take note of include:
A. General cleanliness of the facility and staff. Take into account the
time of day, current activity level, and cleaning/maintenance in
progress.
B. Presence of pervasive odors. Is the odor in one location or
distinctive throughout. Are attemps being made to eliminate transient
odors?
C. Attitude and demeanor of both staff and residents. Do employees as
well as residents look happy? Is staff cooperative and helpful? Are
residents being talked to and having interaction with the staff?
3. WHAT IS STAFFING LIKE ON ALL THREE SHIFTS?
Holgate Community always staffs at the level needed to provide the best
possible care to our residents. In addition to our Licensed Nurse staff
that manages medication administration, we have two Wound Care
Specialist nurses, three Resident Care Managers, two Social Workers, a
Registered Dietician, and a complete Rehab Department of Physical,
Occupational, and Speech Therapy. We also have two Activity Specialists
that plan and coordinate resident activities both in and out of the
facility. At the Oddfellows Home, we have 2 Medication Technicians and
two Care Givers on from 6 AM untill 10:30 PM. During the night, we have
one Medical Technician and one Caregiver on duty with a LPN or RN on
call 24 hours per day. There is also a full time activity specialist
employed Monday through Friday.
4. HOW LONG AM I ALLOWED TO STAY IN A SKILLED NURSING FACILITY?
Length of stay in a Skilled Nursing Facility is dependent on the payor
source for the individual, the medical or rehab needs of the individual,
and the amount of progress made towards indentified goals. MEDICARE will
allow
up to 100 days of skilled care
while HMOs will authorize a various number of days dependent on their
individual policies, progress made, and future potential. An appeal
process is in place should a resident or family member believe that a
denial of further care is unwarranted. Facility Social Workers are available to assist with the appeal process and/or discharge planning for home or a different level of care.
